Dang it.  That entire commit was supposed to be on the branch.

I think the additional test is okay on the trunk, though, especially since it is skipped without __STDC_ISO_10646__.

Add missing files from last commit:

Move Unicode <-> ku/ten mapping into the individual codec modules.
Mapping is based on existing iconv data for single-byte encodings,
and included for several, but not all, multibyte encodings.
