Pull up following revision(s) (requested by tron in ticket #18):

remove stale entries: libcdk was removed 5 years ago, termcap.src between
netbsd-5 and netbsd-6
add "Location" tags which tell where the source lives in the NetBSD
tree, and fix some paths in "Notes" sections
